Dr. Vetamin Veterinary Services Terms and Conditions
Welcome to Dr. Vetamin Veterinary Services, including our Mobile Veterinary Office and Primary and Urgent Care Veterinary Hospital. By utilizing our services, you agree to comply with the following terms and conditions. Please read them carefully.
Booking Policy
1.1 Appointment Scheduling
- Appointments can be made through our website, by phone, or in person at our facilities.
- We recommend booking appointments in advance to ensure availability.
- We retain the right to decline, cancel, or reschedule appointments as we see fit.
1.2 Emergency Visits
- For urgent care, emergency appointments are available and will be prioritized based on the severity of the case.
- Walk-in emergency visits are subject to availability and triage assessment by our medical team.
2. Deposit Policy
2.1 Booking Deposits
- A non-refundable deposit may be required for all appointments to secure the booking. The deposit amount will be communicated at the time of booking.
- The deposit will be applied to the final bill for services rendered.
2.2 Surgical and Specialized Procedures
- For surgeries and specialized procedures, a higher deposit may be required, which will be detailed during the consultation.
3. Cancellation Policy
3.1 General Appointments
- Cancellations must be made at least 24 hours prior to the scheduled appointment time.
- Cancellations or rescheduling within less than 24 hours of the appointment may result in a “Late Notice Fee” of 86 CAD plus applicable taxes.
3.2 Surgeries and Specialized Procedures
- Cancellations for surgeries and specialized procedures must be made at least 48 hours in advance.
- Failure to cancel within this timeframe will result in the forfeiture of the deposit.
- By booking an appointment by any means including via phone, online system, or in person, with Dr. Vetamin, you consent to receive updates, appointment reminders, and pet health-related notes via email from us. These communications are intended to enhance the quality of care and service we provide. If you do not wish to receive such emails, you must notify us in writing either by email or regular mail. Please include your full name and the name of your pet in your request to ensure we can process it accurately.
4. Overnight Hospitalization
4.1 Veterinary Technician and Assistants On-Site
- During overnight hospitalization, only our veterinary technicians and/or veterinary assistants will remain on site to monitor and care for hospitalized pets.
- Our veterinarian will be on call and available for emergency situations, ensuring that any urgent medical needs are promptly addressed.
4.2 Emergency Protocols
- In the event of an emergency during overnight hours, our on-site staff will immediately contact the on-call veterinarian.
- Pet owners will be notified of any emergencies or significant changes in their pet’s condition.
5. General Terms
5.1 Payment Policy
- Full payment for services is required at the time of service unless otherwise arranged.
- We accept various forms of payment, including cash, credit/debit cards, and certain pet insurance plans.
5.2 Service Refusal
- Dr. Vetamin Veterinary Services reserves the right to refuse service to any client or patient if we believe that providing service would be unsafe or otherwise inappropriate.
5.3 Medical Records
- Medical records are the property of Dr. Vetamin Veterinary Services. Copies can be provided to clients upon request.
5.4 Liability
- Dr. Vetamin Veterinary Services is not liable for any indirect, consequential, or incidental damages arising from the use of our services.
5.5 Telemedicine
- Appointments may be conducted by our veterinary technician team, with our veterinarian available through telemedicine platforms such as Zoom, Teams, phone calls, or other remote communication methods.
- Telemedicine appointments have limitations, and you accept these limitations when booking and participating in such appointments.
6. Client Responsibilities
6.1 Accurate Information
- Clients must provide accurate and complete information about their pets’ medical history and any known health issues.
6.2 Compliance with Veterinary Advice
- Clients are expected to comply with the veterinary advice and treatment plans provided for their pets.
6.3 Supervision of Pets
- Clients are responsible for the supervision and control of their pets while on the premises of our facilities.
7. Changes to Terms and Conditions
7.1 Modifications
- Dr. Vetamin reserves the right to modify these terms and conditions at any time.
- Clients are encouraged to visit our website regularly familiarize themselves of any significant changes to the terms and conditions.
8. Termination of Veterinarian-Patient-Client Relationship (VPCR)
8.1 Termination Rights
- Dr. Vetamin Veterinary Services reserves the right to terminate the Veterinarian-Patient-Client Relationship (VPCR) in accordance with the guidelines set forth by the College of Veterinarians of Ontario (CVO).
- Termination may occur under circumstances including, but not limited to, non-compliance with veterinary advice, abusive or threatening behavior, or failure to meet financial obligations.

Vetamin Health Plans
Terms and Conditions for Vetamin Health Plans
Effective Date: January 2025
1. Introduction Welcome to the Vetamin Health Plans provided by Dr. Vetamin Primary and Urgent Care Veterinary Hospital and Dr. Vetamin Mobile Veterinary Office (“we,” “us,” “our”). These terms and conditions (“Terms”) outline the rights and responsibilities of members (“you,” “your”) who subscribe to our health plans. By subscribing to any Vetamin Health Plan, you agree to these Terms. If you do not agree, you must not enroll in a plan.
2. Plan Tiers and Coverage
Vetamin Plus Health:
- Free core vaccines for adult pets only (Canine: Rabies, DHPP, Leptospirosis; Feline: Rabies, FVRCP).
- 10% discount on delivery fees for medication or food (only limited to the products offered by Dr. Vetamin Facilities).
- 10% discount on annual parasite prevention medications. Only when bought for 12 months’ use at the same time (only limited to the products offered by Dr. Vetamin Facilities).
- 5% discount on hospitalization fees. Only once a year and limited to 24 hours. Only at Dr. Vetamin Primary & Urgent Care Veterinary Hospital in Ottawa, ON, Canada.
Vetamin Prime Health:
- Members receive a 10% discount on veterinary exam fees, applicable during the plan’s active period..
- Free core vaccines for adult pets (Canine: Rabies, DHPP, Leptospirosis; Feline: Rabies, FVRCP).
- 10% discount on delivery fees for medication or food (only limited to the products offered by Dr. Vetamin Facilities).
- 15% discount on annual parasite prevention medications. Only when bought for 12 months’ use at the same time (only limited to the products offered by Dr. Vetamin Facilities).
- 10% discount on selected diagnostic tests.** (Radiography up to 6 radiograph images per year, Ultrasound AFAST – two times per year, Advanced imaging – once a year, Comprehensive blood work – two times a year, Comprehensive urinalysis – two times a year, other in-house tests – once a year).
- Waived dispensing fees for up to 3 prescriptions per year (only for medications dispensed by Dr. Vetamin Facilities based on availability).
- 10% discount on hospitalization fees for up to 48 hours per year. Only at Dr. Vetamin Primary & Urgent Care Veterinary Hospital in Ottawa, ON, Canada.
- Eligible for Dr. Vetamin Payment Plan (50% upfront, remaining balance in 3 monthly interest-free installments).
Vetamin Umbrella:
- Members receive a 15% discount on veterinary exam fees, applicable during the plan’s active period.
- Free core vaccines for adult pets (Canine: Rabies, DHPP, Leptospirosis; Feline: Rabies, FVRCP).
- Free delivery for medications and food (only limited to the products offered by Dr. Vetamin Facilities).
- 15% discount on annual parasite prevention medications. Only when bought for 12 months’ use at the same time (only limited to the products offered by Dr. Vetamin Facilities).
- 15% discount on selected diagnostic tests.** (Radiography up to 10 radiograph images per year, Ultrasound AFAST – three times per year, Advanced imaging – once a year, Comprehensive blood work – two times a year, Comprehensive urinalysis – three times a year, other in-house tests – once a year).
- 15% discount on hospitalization fees for up to 72 hours per year. Only at Dr. Vetamin Primary & Urgent Care Veterinary Hospital in Ottawa, ON, Canada.
- 15% discount on dental COHAT (Comprehensive Oral Health Assessment and Treatment). Only at Dr. Vetamin Primary & Urgent Care Veterinary Hospital in Ottawa, ON, Canada.
- Eligible for Dr. Vetamin Payment Plan (50% upfront, remaining balance in 4 monthly interest-free installments).
Vetamin Shield:
- Members receive a 20% discount on veterinary exam fees, applicable during the plan’s active period.
- Free core vaccines for adult pets (Canine: Rabies, DHPP, Leptospirosis; Feline: Rabies, FVRCP).
- Free delivery for medications and food (only limited to the products offered by Dr. Vetamin Facilities).
- 20% discount on annual parasite prevention medications. Only when bought for 12 months’ use at the same time (only limited to the products offered by Dr. Vetamin Facilities).
- 15% discount on all diagnostic tests.** (Radiography up to 20 radiograph images per year, Ultrasound AFAST – five times per year, Advanced imaging – twice a year, Comprehensive blood work – four times a year, Comprehensive urinalysis – five times a year, other in-house tests – three times a year).
- 20% discount on hospitalization fees (up to 120 hours per year). Only at Dr. Vetamin Primary & Urgent Care Veterinary Hospital in Ottawa, ON, Canada.
- 25% discount on dental COHAT (Comprehensive Oral Health Assessment and Treatment). Only at Dr. Vetamin Primary & Urgent Care Veterinary Hospital in Ottawa, ON, Canada.
- Eligible for Dr. Vetamin Payment Plan (50% upfront, balance in 5 monthly interest-free installments).
Notes:
- *Core vaccines are limited to Rabies, DHPP, Leptospirosis, and FVRCP for adult pets only.
- **Selected diagnostic tests include only the tests we are able to perform at our hospital or mobile veterinary office. Reference lab tests and other outside tests are not included. If advanced imaging (abdominal ultrasound and echocardiography) is covered under your selected tier, the specialist review consult is not included in the discount. Diagnostic tests have limits on usage per year starting from the date of your subscription plan.
- Benefits are not applied to any services or products provided by Dr. Vetamin Facilities predating the enrollment in the plans.
3. Membership Terms
- Membership begins upon receipt of the first payment and is renewed monthly or annually based on your selection.
- Coverage benefits are only applicable to services provided at Dr. Vetamin-associated facilities.
4. Payments
- Payments are due monthly (or annually based on your selection) and must be made via the payment methods provided on your account.
- Late payments may result in suspension or termination of plan benefits until outstanding amounts are cleared.
5. Cancellation Policy
- You may cancel your subscription at any time by notifying us in writing or through your online account.
- Upon cancellation, no refunds will be issued for the current month’s payment. Benefits will remain active until the end of the paid period. On annual payments, only the remaining months of the plan are refundable.
- Re-enrollment after cancellation may require payment of an administrative fee.
6. Dispute Resolution
- Any disputes related to these Terms will be resolved through mediation. If mediation fails, disputes will be subject to binding arbitration in accordance with the laws of Ontario.
- Members agree to notify us in writing of any disputes within 30 days of the issue arising. Failure to do so may result in a waiver of the claim.
- Both parties agree to share arbitration costs equally unless otherwise determined by the arbitrator.
7. Plan Changes and Updates
- We reserve the right to modify plan benefits and Terms with 30 days’ written notice. Continued use of the plan after changes constitutes acceptance.
- All updates will be communicated via the email address provided at registration.
8. Limitations and Exclusions
- Benefits are not transferable between pets or owners.
- Discounts cannot be combined with other promotional offers.
- Benefits do not cover grooming, boarding, or non-medical services unless explicitly stated.
- Services must be scheduled in advance and are subject to availability.
- All diagnostics, treatment procedures, and dispensing of medications are performed at the discretion of the attending veterinarian in accordance with the Veterinarian Act and the policies of the College of Veterinarians of Ontario (CVO). Owners may not request diagnostics, treatments, or medications outside of this discretion.
- Unused benefits are not transferable to future periods, other pets, or other owners.
9. Privacy Policy
- We are committed to protecting your personal information.
10. Termination by Provider
- We reserve the right to terminate your subscription for violations of these Terms, fraudulent activity, or misuse of plan benefits. In such cases, no refunds will be issued.
- Members whose plans are terminated due to fraud or misuse may be subject to additional penalties, including legal action.
- If the Veterinarian-Client-Patient Relationship (VCPR) is nullified by either party, the enrollment in the plans will also be terminated according to the cancellation terms outlined elsewhere in this policy.
